Water hardness is the combined concentration of dissolved calcium and magnesium ions, which enter groundwater as it dissolves limestone, gypsum, and dolomite rock. Because a water sample can contain calcium and magnesium in any ratio, labs convert both to a single, comparable number: the equivalent concentration of calcium carbonate (CaCO₃) that would produce the same total hardness. That is why hardness is almost always reported "as CaCO₃" even though the water itself contains no calcium carbonate at all — CaCO₃ is simply the common reference compound.
Why there are five different units for the same measurement

Reading the soft-to-very-hard classification

The U.S. Geological Survey scale classifies water as soft (0–60 ppm), moderately hard (60–120 ppm), hard (120–180 ppm), or very hard (above 180 ppm).
