VLDL cholesterol is rarely measured directly on a standard lipid panel — instead, it is estimated from triglycerides using a simple formula. This calculator applies that formula and flags the triglyceride level at which the estimate stops being reliable.

How the VLDL Calculator works

VLDL is estimated as triglycerides divided by 5, when triglycerides are measured in mg/dL. This approximation comes from the same 1972 paper by Friedewald, Levy, and Fredrickson that introduced the widely used Friedewald LDL equation (LDL = Total Cholesterol − HDL − Triglycerides/5). The underlying assumption is that VLDL particles carry a fairly consistent proportion of a person's circulating triglycerides — roughly one-fifth by weight — under typical fasting conditions.

Because the formula is a fixed ratio rather than a direct measurement, it works best when triglycerides are in a normal-to-moderately-elevated range. This calculator applies the conventional 400 mg/dL cutoff: below that level, the ÷5 approximation is considered reasonably accurate; at or above it, VLDL particle composition shifts enough that the fixed ratio breaks down.

Inputs and what they mean

The only input is triglycerides, entered in mg/dL or mmol/L via the unit toggle (1 mmol/L ≈ 88.57 mg/dL). This value should come from a fasting lipid panel — triglycerides can rise sharply after a meal, and a non-fasting sample will inflate both the triglyceride reading and the resulting VLDL estimate.

The output, VLDL, is displayed in whichever unit you selected for the input (mg/dL or mmol/L, using the standard cholesterol conversion factor of 38.67). Because VLDL scales linearly with triglycerides in this formula, triglycerides is the only variable that can move the result.

Limits and edge cases

The triglycerides ÷ 5 formula is a widely used approximation, not a direct laboratory measurement of VLDL. It should not be relied on when triglycerides are at or above 400 mg/dL — at that level, chylomicrons and other lipoprotein changes distort the fixed ratio, and the Friedewald-family equations built on this same assumption (including LDL) also become invalid. In that situation, a direct VLDL measurement, an expanded or specialty lipid panel, or a non-fasting sample analysis with a different equation is preferred.
