The PaO2/FiO2 (P/F) ratio is the bedside oxygenation measurement clinicians use to stage the severity of acute respiratory distress syndrome (ARDS) under the Berlin Definition. This calculator computes the ratio from a patient's arterial blood gas and classifies it into the mild, moderate, or severe tier.
How the P/F ratio works
The P/F ratio is simply PaO2 divided by FiO2. PaO2 comes directly from an arterial blood gas; FiO2 is the fraction of oxygen the patient is breathing, from 0.21 on room air up to 1.0 on 100% supplemental oxygen. Because the ratio normalizes PaO2 against how much oxygen the patient is receiving, it gives a more comparable measure of lung function than PaO2 alone — a PaO2 of 90 mmHg means something very different on room air versus on 90% FiO2.
The Berlin Definition (ARDS Definition Task Force, JAMA 2012) uses the ratio to stage ARDS severity: mild is 200 to 300, moderate is 100 to 200, and severe is 100 or below. Lower ratios indicate more severe impairment of gas exchange.
Inputs and what they mean
PaO2 (mmHg) is the arterial oxygen partial pressure measured from a blood gas. Normal room-air values are roughly 80-100 mmHg; values are read directly off the lab result, not calculated.
FiO2 (fraction) is the concentration of oxygen the patient is breathing, as a decimal. Room air is 0.21; a nasal cannula, mask, or ventilator delivers a higher FiO2 that should be documented at the time the blood gas is drawn. Using the wrong FiO2 — for example, forgetting the patient is on supplemental oxygen — will produce a misleadingly high or low ratio.
Limits and edge cases
The P/F ratio is only one of four Berlin Definition criteria. A P/F ratio at or below 300 does not by itself diagnose ARDS — the patient must also have symptom onset within one week of a known clinical insult, bilateral opacities on chest imaging not fully explained by effusions or collapse, and respiratory failure not fully explained by cardiac failure or fluid overload. All three severity tiers additionally require a minimum PEEP (or CPAP) of 5 cmH2O at the time PaO2 is measured — a ratio measured without adequate PEEP can understate true lung function. This calculator does not verify PEEP, imaging, timing, or cardiac status; it computes the oxygenation ratio only, and the final ARDS diagnosis always requires a clinician's full assessment.
