Weight-based (mg/kg) dosing keeps a child's medication proportional to their size — but a purely linear formula has no upper bound. Multiply a dose rate by a large enough weight and the 'weight-based' number can climb past what any adult would ever be given. This calculator is built around that specific gap: it takes a generic mg/kg order, a per-dose ceiling, and a max-per-day ceiling, and always shows the number that is actually safe to give — the lower of the weight-based math and the ceiling — rather than an uncapped number next to a warning.

Linear math needs a stopping point

mg/kg dosing scales a dose to body weight so that children of different sizes receive a proportional amount. That works well across the pediatric weight range, but the formula itself doesn't know when to stop scaling. Adult dosing information exists precisely because, past a certain weight, further scaling stops being appropriate — the adult dose (or another absolute ceiling) becomes the effective maximum. This calculator treats that ceiling as a first-class input, not an afterthought: enter it, and the displayed dose is clamped to it automatically.

Two independent ceilings: per-dose and per-day

A single-dose ceiling and a daily-total ceiling protect against different failure modes. A per-dose ceiling stops any one administration from being too large; a daily ceiling stops a frequent regimen from adding up to too much even if each individual dose looks fine. This calculator applies both, independently, and its verdict explicitly says whether the per-dose ceiling, the daily ceiling, both, or neither is the binding constraint for the entered order — this is the round-15 'weight-band tiers plus an absolute max-dose cap' requirement made concrete.

Reading the weight-band reference table

The Max-Dose Ceiling tab's table walks a fixed set of representative weights through the current dose rate, frequency, and ceilings, marking every row where a ceiling changes the result. It also reports the exact weight at which the per-dose and daily ceilings are first reached (ceiling ÷ dose rate). Use it to sanity-check an order across a range of patients, or to see at a glance how much headroom a given rate has before the ceiling takes over.

What this calculator is not

This tool has no drug database. It does not know the correct mg/kg rate, per-dose ceiling, or daily ceiling for any specific medication, age, indication, or patient — those numbers come from the prescriber's order and an authoritative drug reference, and the calculator only performs the clamping arithmetic on the numbers you supply. It is not a substitute for a pharmacist or prescriber's verification, and the ceilings it enforces are a safety guardrail on the math, not a clinical recommendation about what the ceiling should be.
