Estimated total blood volume is a reference figure used in transfusion medicine and fluid management, most often to calculate allowable blood loss or guide replacement volumes. This calculator implements Nadler's formula, the most widely cited method for predicting blood volume from height, weight, and sex.

How Nadler's formula works

Nadler's formula was derived from direct blood volume measurements in 155 normal adults, using a radioactive tracer dilution method (I-131-labeled human serum albumin). The measured volumes were fitted against height-cubed and weight, producing separate equations for men and women:

Male: BV(L) = 0.3669 × height(m)³ + 0.03219 × weight(kg) + 0.6041
Female: BV(L) = 0.3561 × height(m)³ + 0.03308 × weight(kg) + 0.1833

The height term is cubed and expressed in meters (not centimeters) because blood volume scales more closely with body volume than with height alone — a cubic term better approximates that relationship than a linear one. The formula's coefficients were fit specifically to this dataset, so height must be converted to meters before cubing for the formula to produce a sensible result.

Why sex-specific constants matter

Men and women with the same height and weight tend to have somewhat different blood volumes, largely due to differences in average body composition (proportion of lean mass to fat mass). Nadler's formula accounts for this with separate coefficients for each sex rather than a single unisex equation — using the wrong sex's constants will shift the result by several hundred milliliters.

Limits and edge cases

Nadler's formula is a population-derived estimate, not a direct measurement, and it has known limitations. It tends to overestimate blood volume in obesity, because adipose tissue has a lower blood-volume-to-weight ratio than lean tissue — clinicians often substitute an adjusted or ideal body weight when applying the formula to obese patients. It was derived from and validated in adults, so it should not be applied to children or infants, who require separate pediatric formulas. It also does not account for physiologic states that shift blood volume, such as pregnancy, dehydration, or heart failure. When precision matters clinically, this estimate should be interpreted alongside vital signs, laboratory values, and direct clinical assessment rather than used as a standalone number.
